The short answer: Omega-3 fatty acids (EPA and DHA) are not just anti-inflammatory fats. Your body converts them into specialized pro-resolving mediators (SPMs), including resolvins, protectins, and maresins, which actively shut down inflammation and clean up damaged tissue. Without enough of these raw materials, inflammation lingers instead of resolving, quietly driving chronic disease.
I have spent over 30 years in clinical practice watching what happens when patients finally correct their omega-3 status. The changes go far beyond what most people expect from a “fish oil supplement.” Joint pain improves. Brain fog lifts. Lab markers for chronic inflammation start dropping. And the reason is not just because omega-3s fight inflammation. It is because they build your body’s own system for finishing the inflammatory process.
Why “Anti-Inflammatory” Misses the Point
Most people think of inflammation as something that needs to be turned off. Take an anti-inflammatory, suppress the response, move on. But inflammation is not a switch. It is a process with a beginning, a middle, and an end.
The beginning is necessary. When you are injured, infected, or exposed to a toxin, your immune system launches an inflammatory response to deal with the threat. That part is healthy. The problem is when the process never completes. When the fire keeps burning because your body lacks the molecules to put it out.
This is where the conventional approach falls short. NSAIDs and other anti-inflammatory drugs suppress the inflammatory response. They starve the fire of fuel. But they do not actively resolve it. They do not clear the debris. They do not restore normal tissue function. The difference matters more than most people realize.
What Are Specialized Pro-Resolving Mediators?
In the early 2000s, researcher Charles Serhan at Harvard Medical School made a discovery that changed how we understand inflammation. He identified a class of signaling molecules that the body produces specifically to complete the inflammatory process. He called them specialized pro-resolving mediators, or SPMs.
There are three main families of SPMs:
Resolvins, derived from both EPA and DHA, actively shut down the inflammatory response once its job is done. They signal immune cells to stop producing inflammatory chemicals and stand down.
Protectins, derived primarily from DHA, protect tissues from collateral damage during inflammation. They are especially concentrated in the brain and nervous system.
Maresins, also derived from DHA, orchestrate the cleanup. They promote a process called efferocytosis, where immune cells clear dead cells and damaged tissue so healing can begin.
Think of it this way. If inflammation is a fire, most supplements and medications try to starve it of fuel. SPMs are the fire department. They put the fire out, clean up the wreckage, and restore the neighborhood.

EPA and DHA: The Raw Material Your Body Needs
EPA and DHA are the active, long-chain omega-3 fats found in high-quality fish oil. Unlike plant-based ALA from flax or chia, which the body converts poorly (often less than 5%), EPA and DHA are used directly by your cells.
Each one does different work.
EPA primarily supports inflammation control, cardiovascular health, mood regulation, and triglyceride reduction. It is one of the direct precursors for resolvin production.
DHA is structural. It concentrates in the brain, retina, and cell membranes, supporting cognitive function, memory, and vision. It serves as the raw material for both protectins and maresins.
Together, they also improve cell membrane fluidity. This is a detail that rarely gets discussed but has enormous clinical implications. The fluidity of your cell membranes affects how well insulin, hormones, and nutrient signals actually reach the inside of the cell. Stiff, omega-3-depleted membranes mean sluggish signaling across every system.
A 2019 study published in Circulation Research confirmed that enriched marine oil supplements increase peripheral blood SPM concentrations and reprogram host immune responses, providing direct evidence that supplementing with EPA and DHA translates into higher SPM production in the body.
Why This Matters for Chronic Disease
The implications extend far beyond joint pain or general “inflammation.”
Chronic, low-grade inflammation is the common thread in most of the conditions I see in practice. Insulin resistance. Metabolic syndrome. Cardiovascular disease. Cognitive decline. Autoimmune conditions. Gut dysfunction. These are not separate diseases with separate causes. They are different expressions of the same underlying problem: inflammation that never fully resolves.
When inflammation lingers, it blunts insulin signaling, damages mitochondrial efficiency, and keeps cortisol elevated. Those three factors alone explain why so many patients struggle with stubborn weight gain, low energy, and poor recovery despite doing “everything right.”
Supplementing with high-quality EPA and DHA gives your body the raw material to actively resolve that inflammation rather than just suppress it. You are not masking symptoms. You are supporting a cleaner-burning metabolic terrain.
What Most Fish Oil Supplements Get Wrong
Not all omega-3 supplements are equal. Most commercial fish oil is low-potency, poorly purified, and oxidized before it reaches the shelf. Oxidized fish oil can actually increase inflammation, which is the opposite of what you are trying to accomplish.
What matters in a quality omega-3 supplement:
Concentration of EPA and DHA. Many products advertise “1,000mg fish oil” on the label but contain only 300mg of actual EPA and DHA combined. The rest is filler fat. You need the active compounds, not the carrier oil.
Third-party testing. Heavy metals, PCBs, and oxidation markers should all be verified by an independent lab. If the manufacturer cannot show you a certificate of analysis, that is a red flag.
Molecular form. Triglyceride-form omega-3s absorb significantly better than ethyl ester forms. Many cheaper supplements use the ethyl ester form because it is less expensive to produce.
SPM content. Some advanced formulations now include pre-formed SPMs alongside the EPA and DHA, giving your body both the raw materials and the finished product. The Omega-3 Deficiency Most People Do Not Know They Have
The standard American diet is critically low in omega-3s. The ratio of omega-6 to omega-3 fats in a typical modern diet runs anywhere from 15:1 to 25:1. Our bodies evolved on a ratio closer to 2:1.
That imbalance matters because omega-6 fats promote inflammatory signaling while omega-3s promote resolution. When the ratio is skewed that far toward omega-6, your body is constantly generating inflammatory signals without the counterbalancing resolution signals.
This is not a minor nutritional gap. It is a systemic imbalance that affects the brain (DHA is the most abundant fat in neural tissue), the cardiovascular system, the gut lining, joint tissue, and virtually every cell membrane in the body.
In my clinical experience, correcting omega-3 status is one of the highest-leverage interventions available. It does not fix everything. But it creates the conditions where everything else you do, from dietary changes to exercise to stress management, works better.
When to Talk to a Practitioner
If you are dealing with persistent inflammation, joint pain, brain fog, metabolic issues, or fatigue that does not resolve despite lifestyle changes, it may be time to look deeper. Standard blood work rarely checks omega-3 status or inflammatory resolution markers.
Functional lab testing can reveal what is actually happening at the cellular level, including your omega-3 index, inflammatory markers like high-sensitivity CRP, and the metabolic patterns that suggest unresolved chronic inflammation.
